For future reference:

1) Temp cards filled out with wrong companies insurance info and wrong ID.
2) Prescriptions rejected for "Not enough reciept and written in information in its place." Entire reciept was sent in and missing info hand written as per their own instructions.
3) Wrong mailing address on file with LabCorp, rejected as this went to another insurance company.
